What services are offered?

Physical Therapy & Rehabilitation Services

This program offers comprehensive physical therapy services and personalized at-home exercise plans for conditions such as:

- Muscle and joint pain
- Back and neck pain
- Weakness
- Gait and balance disturbances
- Repetitive use injuries, such as tendonitis
- Services across the lifespan
- Sports rehabilitation and injury prevention

IMPORTANT NOTE: The Physical Therapy & Rehabilitation Services at Hart-Wood are by clinician-referral ONLY. You can come with outside PT scripts, or you will need to be seen by the WFCC medical team first.

We are occasionally closed in the event of snow, severe weather, or holidays.

What do I need to bring in with me to the clinic?

The Physical Therapy & Rehabilitation Services at Hart-Wood are by clinician-referral ONLY. You can come with outside PT scripts or you will need to be seen by the WFCC medical team first.

What if the medical team cannot see me during the clinic time?

If the clinicians are at capacity, physical therapy treatments may need to be rescheduled for a future appointment.

What if I come without a PT script from a healthcare provider?

You will need to be seen by the WFCC medical team first, as this program is by clinician-referral only.
